Northwest Portland Physical Therapy - Part Time Physical Therapy Aide

We are currently seeking a part time physical therapy aide to join our team! The PT aide performs a variety of tasks including patient care (ultrasound, application of hot/cold packs), cleaning (patient rooms, laundry, exercise equipment), front office assistance (scheduling patients, answering phones, faxing, filing, ordering supplies) and various other projects as needed.

Our clinic is located in the Montgomery Park building in NW Portland. Our services include outpatient physical therapy, massage therapy, and fitness center for the building. We have two physical therapists and two front office personnel.

The ideal candidate would have the following qualifications: strong ability to multitask, thoroughness, punctuality, excellent communication skills and self-motivation. This job is perfect for a student interested in a career in physical therapy/exercise science.

The position is part time, roughly 10-15 hours per week. We are looking at roughly 4-7PM M-F with additional flexibility to cover extra hours if needed.

Summary:  The program manager is responsible for the day to day supervision and operation of Alcohol and Drug Free Transitional Housing Programs and Resident Service Programs within the Supportive Housing Department. Essential duties and responsibilities:

  1. Supervise ADFC Case Managers and Resident Service Coordinators and ensure client housing plans, recovery plans and over all case management plans are being developed and adhered to.
  2. Monitor length of stay in programs in accordance with building/program policy.
  3. Monitor program discharges, financial obligations and disciplinary action.
  4. Monitor, and assist as appropriate, staff efforts to connect residents with appropriate services with case plans

Description of other duties:

5.      If in recovery, must have a minimum of two years sobriety.

6.      Must be able to follow enrollment, screening, assessment and case management planning protocol. 

7.      Must be able to work under stressful conditions.

8.      Must be able to be on-call on rotating basis.

9.      Must possess a current drivers license, pass a DMV background check and be designated an “acceptable” driver as outlined in Central City Concern’s Fleet Safety Policy.  Must pass an initial drivers training and then pass an annual recertification.

Program DescriptionThe Community Engagement Program (CEP) is a comprehensive, multidisciplinary recovery model designed to meet the needs of chronically homeless individuals with co-occurring mental health, addictive disorders and/or physical healthcare concerns. This underserved population has failed to respond to most intervention efforts due to the debilitating effects of primary and mental health issues, addiction and the isolating nature of homelessness.  CEP works with individuals in their communities utilizing a Stage of Change approach to treatment to move participants towards self sufficiency. 

Summary:

The CEP Clinical Supervisor is responsible for the daily clinical supervision of the Community Engagement Program (CEP) team.  This position oversees an array of clinical services to clients of CCC appointed to the CEP team.  Those areas include Intensive Outreach, Community Support, Chemical Dependency Treatment, Mental Health Treatment, Physical Healthcare Treatment, Prescribing, Case Management, Accessing appropriate and affordable housing, benefits and entitlements, and working collaboratively with inter and intra agency partners on the provision of services needed in all life domains to promote recovery and self sufficiency.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

-          Monthly  individual supervision with clinical case management staff

-          Monthly group supervision

-          Review and sign off on assessments

-          Review and sign off on treatment plans (assuring that treatment plan goals are tied to assessment)

-          Review progress notes with staff for clinical appropriateness; stage of change and appropriate corresponding stage of treatment and assuring stage of change and stage of treatment documentation.

-          Provide in-service trainings on clinical requirements; assessments and treatment.

-          Observe mental health and A&D groups and provide feedback on facilitation to group leaders as appropriate.

-          Assist other supervisory staff and mental health specialist with review of high risk cases and the development of crisis plans

-          Assist with critical incident reports and critical incident debriefings

-          Maintain documentation of staff charts (out of compliance issues) and report to program supervisor and/or program manager.-          Work with program manager and program supervisor to expand and maximize service delivery

-          Participate in QI/QIC meetings.  Work with program manager and supervisor to ensure all aspects of clinical work is in compliance with ISSR’s and CCC compliance standards.

-          Will be required to carry an agency cell phone including after hours on-call rotation

-          Perform other duties as assigned.

SUMMARY:  Old Town Clinic is a busy primary care practice and Federally-designated Healthcare for the Homeless program. The clinic
serves more than 2,600 patients per year, most of whom are experiencing poverty and many experiencing homelessness. They present with a variety of medical and behavioral health conditions.  This position is a Clinical and Nursing supervision role.  The purpose of the po
sition is to plan, organize and supervise clinical support and nursing care. This position directly supervises the Panel Managers, Medical Assistants, and Health Assistants. The Clinical Supervisor will provide guidance and supervision regarding clinical and nursing practice issues, and to assure the appropriate review and observation of nursing, medical, and related clinical activities. The Clinical Supervisor will lead quality improvement activities to enhance the efficacy and efficiency of clinical operations, as well as assuring that clinical operations are consistent with best practices and community standards.  The Clinical Supervisor is part of the Clinic Management Team and will participate in weekly meetings regarding clinic operations and planning.

Essential Duties and responsibilities:

1.     Supervision and management of staff.

  1.     Directly supervise Clinical Support staff, including: Panel Managers, Medical Assistants, Administrative Medical Assistants, and Health Assistants.

  2.     Plan, prioritize, assign, supervise and review the work of clinical support staff.

  3.     Ensure adequate staffing of clinical support services; arrange coverage for extended absences.

  4.     Directly observe practice, skills, client interaction, and review staff documentation.  Conduct performance evaluations. Coach, counsel and discipline for performance.

  5.     Work with Clinic leadership to assure the continuity of care and compliance with the rules of licensure and practice. Assures the delivery of evidence based interventions in the care of clients.

  6.     Review applications, interview and hire assigned personnel in accordance with the policies and procedures of HR. Assures appropriate training and coaching has occurred for new staff, on-call staff, students

  7.     Work with Medical Assistants to achieve proficiency on MA Competencies and develop/maintain a system to track proficiency achievement.

2.     Quality Improvement and Quality Assurance

  1.     Plan, lead, and participate in quality improvement activities that enhance the efficacy and/or efficiency of clinical services.

  2.     Create and maintain systems to assure clinical services and supprt functions are in compliance with all applicable rules and regulations, and consistent with community standards of practice.

  3.     Identify best practices that can be incorporated into clinical operations, and work with Clinic leadership to implement these practices.

  4.     Review, investigate, and respond to all client grievances related to clinical services or clinical staff. 

3.     Administrative duties:

  1.     Prepare agendas and conduct staff meetings.

  2.     Maintain time and attendance records.

  3.     Prepare memos, letters, reports, notices, forms, and procedural guidelines.

  4.     Participate as a member of the clinical leadership team for decision making and coordination.

  5.     Ensure personnel and materials for clinical support are within budgeted amounts. Notify the Director of Healthcare as soon as possible regarding anticipated over-expenditures.

Additional Duties:

Oregon Youth Authority.
 

Position(s):                 Youth Counselor Positions:  one part-time position at our Son Village Program and relief staff positions at both Son Village and Charis Ridge.                         Job Description:           As a team member, you will provide counseling and supervision to the youth who live in the program, perform assigned tasks and do the required documentation. Son Village serves up to 15 boys (ages 13-17) and is located in Welches, 10 minutes east of
Sandy. Charis Ridge serves up to nine boys and is located in Troutdale. Both of these positions will provide excellent experience for those who have a heart for at-risk youth and/or are considering working with youth as a career.

Work Schedule:         The regular position is 9 hours/week, 10pm Saturday-7am Sunday.  This is a great position for a student as there is time available for study. The other is an on-call (relief) position where the person can work as much or as little as they wish, but set hours are not guaranteed.

Qualifications:           Preferably enrolled in or having completed Bachelor’s degree in the area of psychology, juvenile justice, sociology, human services, youth ministry or have had some experience working with youth.  Looking for a mature person with patience, strength of character and good relational skills.

Wage:                         Persons with BA degree start at 9.50/hr. with consideration for experience. Full-time positions include benefit package of health/life insurance, vacation/sick/holiday pay.

 SHIFT:                      Monday through Friday, 3:30pm to 8:30pm  SUMMARY:  Treatment Assistants are responsible for providing care and close intensive supervision to all clients and their children around the clock. General responsibilities include: observing and reporting behaviors, helping to implement the treatment plans developed by counselors, group management, and other tasks as needed.  

Essential duties and responsibilities:

1.            Coordinate residents’ activities as detailed on the weekly schedule.

2.            Facility maintenance, including monitoring and supervision of clients’ performance of household chores and assigning weekly household tasks.

3.            Teach and role model care of infants to clients.

4.            Attend staff meetings and trainings.

5.            Answer house phone and perform various clerical duties as needed.

6.            Distribute client and child’s medications and maintain accurate records.

7.            Administer and document observed UAs (urinalysis).

8.            Assist clients in completing paperwork from other agencies.

9.            Transport residents to appointments as needed.

10.          Assist clients with activities specifically related to women; i.e. breast-feeding, feminine hygiene needs, etc.

11.          Assist supervisors in training and supervising TA Is.

12.          Perform other duties as assigned. 

Description of other duties:

1.    Provide care for children and infants while women are in treatment.  Facilitate non-clinical groups and provide chart notes in clients’ clinical records.

2.    Review records for accuracy and maintain filing systems.

Skills & Abilities:

1.            Understanding of chemical dependency as a disease, including treatment and intervention methods.

2.            Ability to work both as a team member and independently.

3.            Ability to learn requirements of HIPPA, confidentiality and boundaries, and incorporate into daily job performance.

4.            Ability to clearly communicate expectations and consequences to clients.

5.            Ability to manage complex and multiple tasks and prioritize appropriately.

6.            Sufficient manual dexterity and physical ability to perform assigned tasks.

7.            Ability to communicate effectively, clearly and concisely both verbally and in writing, with clients, their children, co-workers, corrections personnel, police, child welfare workers and other community members.

8.            Ability to work with people from a diverse range of ethnic and socio-economic backgrounds, including those with dual diagnoses or other disabilities who may display hostility.

9.            Ability to maintain accurate records and necessary paperwork.

10.          Knowledge of and ability to utilize de-escalation techniques, when encountering volatile situations.

11.          High degree of comfort and sufficient advancement in personal recovery to appropriately facilitate clients’ in theirs.

12.          Demonstrated understanding of professional and personal boundaries in the workplace. 

Minimum qualifications including education, experience, training, license and sobriety: 

1.            Two years relevant college coursework or completion of CCC approved education or training program.

2.            At least two years work or volunteer experience in related field required (i.e., shelter, residential or day treatment, counseling, substance abuse treatment, etc.).

3.            Prior experience working with at-risk women encouraged.

4.            Knowledge of Twelve-Step Recovery process.

5.            Have or able to obtain Standard First-Aid, and Adult and Child/Infant CPR Certification.

6.            Individuals who are recovering from a substance abuse related disorder must be able to demonstrate continuous sobriety under nonresidential, independent living conditions for the immediate past two years per OAR Standards.

7.            Physical ability to bend, stoop, kneel, squat, reach, twist, pull and lift heavy objects up to 50 lbs., 3 feet off the ground several times per shift.

8.            Must pass a pre-employment drug screen and criminal background check.

9.            Must possess a current drivers license, pass a DMV background check and be designated an “acceptable” driver as outlined in Central City Concern’s Fleet Safety policy.  Must pass a drivers training and an annual recertification training.

10.          Must adhere to agency’s non-discrimination policies.

11.          Ability to effectively interact with coworkers and clients and children with diverse ethnic and cultural backgrounds, religious views, political affiliations, sexual orientation and treat each individual with respect and dignity.

The Customer Relationship Specialist handles inbound prospect and inquiries via phone, email, and/or chat. Each Specialist will make outbound calls as a result of follow-up on inbound email or chat inquires, marketing directed customer retention campaigns, or other campaigns, and will be measured on several Key Performance Indicators (KPI’s) as an individual contributor and as well as their participation in delivering to the collective sales and service goals of the Call Center.

Responsibilities:       

      Prospect & Inquiry Management Process

·       Achieve assigned annual quota and revenue objectives

·       Utilize effective telephone, email, and account management relationship building strategies

·       Achieve assigned quotas for inbound, outbound event and contact levels

·       Manage daily / weekly tasks towards the Key Performance Indicators

·       Pre-qualify potential customers and schedule appointments for center tours

Adaptability Requirements:                         

Incumbent will be faced with a variety of issues on a daily basis and will be expected to respond appropriately.

Working Conditions:             

Work is primarily sedentary in an office environment. Significant telephone and computer use is required. 

Experience and Education:

·         BA or equivalent liberal arts/business/management experience desired

·         At least 2-4 years of telemarketing relationship sales (B2C or B2B) experience required

·         Formal sales training such as Solutions Selling, Miller Heiman, or Spin is helpful but not required

Knowledge, Skills, and Abilities:     

·       Excellent interpersonal, written and oral communication, and presentation skills

·       Must have patience and stay composed under stress and difficult situations

·       Demonstrated ability to aggregate and analyze multi sourced data

·       CRM Experience (SalesForce.com, Siebel, Onyx, or Pivotal) helpful but not required

·       Demonstrated professionalism, initiative and enthusiasm

·       Proficiency in the use of email and navigating within the internet

Hours of Operation:

            7:30am – 8:00pm: Monday-Friday, 7:30am – 7:00pm: Saturday and Sunday.
